No, it's different, but I can see why it seems that way.
It's two fold...
The actual Unitrends server can only be installed on Hyper-V. But the installer for it only runs on Windows. So if you don't have both Windows and Hyper-V you can't use it. It's insane. Once on Hyper-V, it only runs on CentOS! So three disparate systems all are required to install their one application.

Veeam just runs on Windows, period. There's no "must be on X hypervisor" and no "must have Y OS to install." There's just one requirement.
If Unitrends allowed you to install directly, it would just be "Requires CentOS".

So the official instructions (for the trial not free) say to extract the VHD from the ISO.. But there is an exe in the ISO and I cannot open it with 7zip.
So where the fuck am I supposed to get the VHD from?

The it's probably a self-extracting .exe. Extract the the EXE from the .ISO... then run the EXE, which will probably extract the VHD. Just a guess.

Could it be like this because the .EXE automatically extracts AND configures the VM? If so, then it would make sense to require Hyper-V Role be installed.

If I remember correctly when you start the installation, there’s a temporary folder with multiple powershell scripts that does most of configuration.

That's definitely not a good reason for the role to be needed.
Nothing like requiring a non-production approach! It speaks volumes as to how they see themselves.

Btw, is there anything agentless and free for Hyper-V or vSphere that can be used for scheduled VM backup? I had hopes for the free Unitrends but now they're shattered after this post.

Altaro can and the free version allows you to backup 2 VMs.

Which part do you want free? ESXi/vSphere slash Hyper-V or Veeam?
VBR does offer a free edition, but does not include scheduling capabilities. You can get around that with PS scrips invoking the jobs and Task Scheduler. However, if it breaks, then you get "Best Effort" support.
VMware ESXi/vSphere - No, because the API's will not be enabled for something like Veeam to interact with it..
Hyper-V - If you download the free Microsoft Hyper-V 2016 hypervisor (not a full blown 2016 server and add the Hyper-V role), then it is too free, but can be a pain to manage sometimes, but does allow for you to have VBR interact with it for free. Only downside for it with VBR is that you don't get all of the features that you could get with ESXi/vSphere.
