I don't doubt it was staged. I also don't doubt that it was taken too far. The same things have happened in wrestling and boxing before it.

But are the injuries staged?
Not according to the hospital that treated them. One has corneal abrasions from broken glass. That could be a career ender.

It is not a flaw, but a feature. It is designed to have user input data. I'm totally not shocked.

I guess "vulnerability" is a better word for it. As designed, but introduces a risk.
