      Not sure how many sites you have setup, but is there a possibility that you accidently added it to another site in the controller? I know you mentioned the MAC did not show up when you queried it before. I had an issue while back where I was trying to adopt an old USG I had on hand and it was not showing up because I never removed it from another site.

        @syko24 It has been factory reset multiple times and not in our controller anywhere.

              I know you can point an AP to the IP of the controller. Is that an option here?

                    @CCWTech When you do your set-inform are you using the domain name or IP of the controller? If the domain name, maybe dns is not resolving correctly. Maybe try your inform by IP???

                        Since you're waiting anyway... I'd setup a local controller and see if you remove the inform line if a local controller picks it up on broadcast.

                        But if you have better things to do.. then just wait for the new one.

                          @dashrender Even tried that and it adopts. I tried to then migrate from that local controller to the online one and no luck. I had the ISP make a change (increase MTU) and going to try when the new one arrives tomorrow. Hopefully that will fix the issue.

                                  The provider upped the MTU to over 2000 for us.

                                    The new router arrived and adopted without any issues. It would have been nice to try the old router since the MTU changed so we know which of the two was the fix, but I am happy to report all is working well with the new router in place.

                                    Sending in the old one for RMA credit.

                                      @ccwtech I had a similar issue on a Unifi Switch once ... It took me forever to track the issue down and in the end, it was such a simple thing that pissed me off. Apparently someone had installed some IP cameras on the network and assigned a static IP address that was conflicting with the network switch. A new device would cure you because with a new mac, a new IP. Just something to check out.
