RDSH for a Medical Office

So, I am helping a friend try out RDSH (Server 2016) for his small business. He has an orthodontic practice and had shared a proposal he got form a vegas company that appears to focus on thin client and VDI solutions for medical offices. Looking at the proposal there was 12 thin clients, $10,000 in setup fees, but then it appeared to be a lease agreement that raised the total cost to about $80,000.
I am just helping at the moment, and don't want it to become an obligation until I decide what I am doing career-wise moving forward. We are going to trial Azure and Vultr, both of which I have some experience with. We are also weighing the merits of a local Hyper-V deployment. He is planning to open a second location so keeping it in the cloud would be a real advantage.
Some things I was thinking about...
1.) With my first deployment I ended up with a Vultr instance that provided the Server 2016 license and I purchased RDS CALS. I am assuming its licensed per CPU and per User licenses are not required. Am I correct about this?
2.) Trying to think of what HIPAA implications there might be. Have been using GFI Max (Solarwinds) for backup and that seems to be going well.
3.) For a small deployment, does Citrix really offer anything now that RDP is so well developed. I assume Citrix is more for a farm where App-V is used and there are users roaming to multiple servers.

12 clients? In this case, a VDI doesn't make any sense. For less than 10k$ you can setup a hell of office workstations, with far better performance (especially for latency) and reliability.

VDI is what is being pushed by the current vendor (effortlessoffice dot com) but I am a big RDSH fan as of late. Havent had latency issues with Vultr yet.
For RDSH I dont see how the cost is prohibitive at all. And when using session host its actually easier to manage IMO. You deploy software once. Remote control is built it. Data is local from block storage.
Its almost like RDSH is now one of those best kept secrets for small businesses.

Where will his EHR be hosted? Local or cloud?
I would say it can be done a good bit cheaper that $80k,.. I mean wow. I know NTG could spec it out less.
So much of HIPPA is simple common sense with no all users seem to understand. You don'[t keep patient data on the local computer.
IIRC - Bitlocker isn't 'approved' for drive security - Vera Crypt I know isn't. But if you do an RDS session to the server - keeping all the data on the server, you may be fine with that. Keeping everything off the local prevents multiple failure points.
