Order Management System - OMS

So as we are growing as a business our ordering process, is kind of hap-hazard.
"Oh you need this okay, put in a help desk ticket, or leave me a post-it note"
Which is awful. I'm looking for something simple to setup, simple to use (from a user stand-point) and simple to use from a purchaser stand point.
Simple administration would be a plus as well. I've considered just recommending creating a "Purchase Request Form" which would likely work well enough in most cases, but paper isn't easy to track.
I don't think I need any order integration of any sort, but it would be nice to have a list of items that people would need, that the IT department would fill in (as a check box maybe) and when someone creates an order request they'd specify what needs to be ordered by selecting a check box.
Ideally the goal use for this would be to manage items like print toner, paper stock etc.
Does anyone have any recommendations that might work well?
We currently use spiceworks, but for ticketing only. The other capabilities just aren't utilized. I'm not sure how well they even work.

A web form on google docs... Here fill this out hit submit.. The back end would give you the admin of it, more options.

@DustinB3403 I've generally just used our suppliers user and approval workflow system for this. Insight, Softcat, CDW all have built in OMS functionality.

Ours is mostly manual through Sales people and our Ordering/Quoting system due to the nature of our business and people usually need help with it. (It's mostly business to business).
We do have EDI setup for some of our larger consistent customers who are mostly doing "cookie cutter" projects with some minor changes.
