Workplace safety requirements require protection from falls. Yes. 6 feet or more in most states.
Not full fall gear. In general industry (what most of us work in except maybe @NerdyDad) fall protection is required starting at 4ft. In most instances a ladder is considered fall protection. Guess who just went through a fall training course because I may have to go up on a lift "someday".
Portable ladders are not required by OHSA to have any type of personal fall arrest systems. Fixed ladders do if you are working above 24'.
Some people like to say you have to on portable ladders above 6', but that is not an OSHA ruling.

@scottalanmiller I read somewhere else today that Android phones are by far the most prevalent in U.S. So which is it? Do news sites just say what their readers want to hear?

Most prevalent but not the largest market share by revenue. iPhones earn more, Android sells more. iPhones increased while Androids decreased this past month.
