If you saw the URE error listed in this link (and below) would you take this to mean that you need to rebuild the array and reinstall from the ground up?
Errors detected on drives during previous rebuild or background surface analysis scan. Error Class : 0x000a | Error Code: 0x1716 Symptom Unrecoverable media errors detected on drives. Cause Corrupted data on drive. Action Backup and restore the drives.

The array is showing healthy otherwise in ILO, this is reported for specific sectors of the logical volume. No drives or smart issues are shown.

@DustinB3403 URE always means that you should recreate your whole Array, I would go even further and replace the drives. That said was this RAID5?

@dbeato yes a RAID5, the drives are all passing SMART without issue. It's a rather small installation. I assumed as much as needing to rebuild the entre array and am working on a process to remediate this.

Yes, you absolutely must replace the drive(s) that has read errors.
S.M.A.R.T doesn't do a full test unless you initiate the extended test. Which may takes hours when you have the drive offline and much longer than that if it's in the array. -
@Pete-S I would agree if any of the hardware was being reported as defective. HPE won't just replace drives without an indicator that the drives are the issue.
This appears to be a software issue as the corruption is in the logical drives and not being reported from the physical devices.

Are they SSD or Spinners? If they are spinners this is a great time to get away from RAID5.

LOL - you mean it's a great time to get away from spinners
I know there are still cases for spinners. We can only hope this system is ancient since it's running RAID 5 on spinners, so we should be able to assume the drives are tiny - easily replaced by two SSDs in RAID 1.

I know there are still cases for spinners. We can only hope this system is ancient since it's running RAID 5 on spinners, so we should be able to assume the drives are tiny - easily replaced by two SSDs in RAID 1.
That as well! But at the very least no RAID 5 if you are re-using drives. But all my new server builds are SSD for sure.

Turns out that the embedded controller is defective. HPE has a tech arriving today to replace the defective part.
