New PBX - on prem or off?

they don't claim HIPAA compliance.. so while they are likely OK, we wouldn't use them.

I'd suggest checking out http://mfax.io. They are HIPAA complaint https://www.mfax.io/hipaa-compliant-fax and we get feedback all the time from our customers about how great they are.

Nice - thanks
*EDIT - total brain fart!
I do 700 Pages A DAY. That's more than 14000 a month. Their listed large account is 2500 pages, and costs $80 - the same I pay today... so no, this service would likely cost me 2-5 times more than I pay now...
Below this no longer matters.
This pricing is clearly better than my current pricing... now to see how badly it destroys my workflow, and how do I send outgoing faxes?I still need the "walk up and drop the fax on the machine" option for users.

They offer ATAs too - so you can keep the fax machine on the ground for the users.

2500 pages = $80
each extra = $0.04$80 + (700 * 22 - 2500) * .04 = $596
That's more than 7 times more than I pay now.

meh, It's starting to go down.. we're finally starting to see other technologies get used - like Direct Messaging - i.e. an email from EHR to EHR, but only works on verified senders/receivers.. etc etc etc.. but it's also an XML transfer - so the data is actually live, not just a scanned in paper document... and that causes it's own issues because we as the users, want the different parts actually sorted appropriately, but often we get tons of data we don't care about - so we have to go in and delete that data, or push it off to the side.. where in the fax days, you just ignore those pages of data.

We had our IT team assessed to see where we would need to be to accomplish what we want to accomplish and the company came back with 2 million in salaries alone. We are barebones for what we cover

Damn I just saw a bill from cox.
A single sip phone totally managed by them end to end, $51.00/m
That includes:
The phone
DID
Vmail
Management of the line from the phone all the way to cox.
Calling inside the USA, ie no long distance fees. -
Can't vouch for quality of service but I found this company while looking at t.38 solutions. Cost is per minute instead of per page.
https://www.t38fax.com/pricing/
They offer volume pricing. With your usage it would come out to $350/month just on minutes so still not cost effective but if you could negotiate a lower rate might be worth it. -
